For Doctors
The month between visits, brought into the room
Your patient brings the readings you asked for, their medicines as they logged them, falls in their own words and a check of the house. Every page says where each fact came from, and the clinical judgment stays with you.

You get fifteen minutes together. Between visits, your patient has a month at home: stairs, medicines, meals, routines. That month holds the real context of their health, and it is the hardest part for you to see.
SeniorThrive brings that month into the room, so your patient walks in prepared and ready to partner with you.
They arrive with a few clear pages. The readings you asked them to track: blood pressure, glucose, weight or oxygen.
Two weeks of medicine doses, as they logged them.
Every fall, slip or dizzy spell, in their own words: the room, the lighting, what they were doing, how they got up.
Their CDC STEADI Timed Up and Go, timed at home, beside their last time. And how daily tasks are going, rated by them across the Katz and Lawton domains.
Health also lives in the house. With Room Safety Check, your patient photographs each room. SeniorThrive's read of those photos, informed by CDC STEADI and AOTA home safety principles, flags things like loose rugs and dim stairs.
When it finds a concern, it tells your patient to ask you about an occupational therapy home evaluation. The referral, and the judgment, stay with you.
Outside your clinic, your patient carries an Emergency Card: medicines, allergies, conditions, who to call, and a QR code. Family stays close through their Circle, seeing only what your patient chooses to share.
Every page says where it came from: self-reported, timed at home, or quoted from the FDA label. Nothing is sent to your practice. Your patient brings what they choose.
SeniorThrive is a plan for living well at home. And you stay the clinician guiding their care, with a fuller picture of the month you could not see.
Why doctors share SeniorThrive with their patients
Fifteen minutes, with the month behind them
The readings you asked for. When you ask a patient to track something, the app asks which doctor asked. Blood pressure, glucose, weight and oxygen readings save with the date, ready to print for the visit.
Pages that say where each fact came from. Medicines as logged, falls in their own words, a self-timed Timed Up and Go and self-rated daily tasks. Each page marks what is self-reported, and medicine facts are quoted from the FDA label or MedlinePlus.
The house, with a handoff to you. Room Safety Check reads each room from photos. It tells your patient to ask their doctor about an occupational therapy referral, and the call stays with you.
The best moment to share: the end of a visit.

Between visits
The readings you asked for
When your patient sets up a reading, the app asks whether a doctor asked them to track it, and which doctor. Blood pressure, glucose, weight, oxygen and the other readings save with the date, with a reminder at the time they choose. Each one prints as a report with its average, range and every reading.

Notes for the next appointment
From My Medicines, your patient prints Notes for my next appointment: their current medicines and what each is for, what they have noticed lately in their own words, the three CDC STEADI screening questions already answered, and their home check. From How I'm Feeling, they can also print a report for you: each symptom with its date, how strong it was out of five, what set it off and what helped, over the last 7 or 30 days.

A few months on one page
The Thrive Summary sets mobility timings beside the prior reading, notes changes in daily tasks, and shows medicines as logged over 30 days and vitals as ranges over 90 days. It is marked self-reported and is not a medical record. Medicine History shows two weeks of doses, taken and missed.

What you told them, kept
In My Doctor Visits, your patient writes down which doctor they saw, what you told them and what they need to do. If you set a follow-up, it goes on their calendar in one tap, and they can send the follow-up steps to their family. Notes stay private unless they choose to share them.

A one-page balance and home summary
My Home and Balance Summary prints the patient's answers to the CDC STEADI Stay Independent checklist with their score, their medicines that fall in the CDC STEADI fall-risk classes, falls and near-misses from the past year, the top open items from Room Safety Check, and questions to ask you. It quotes the CDC's own threshold rather than giving a verdict, and says it is self-reported.

Falls, slips and dizzy spells
Balance & Fall Notes records each event: where they were, what they were doing, the lighting, how they got up and how they felt. A note a family member adds is labelled as theirs.

The Emergency Card
A wallet card and a full page with allergies, medicines, conditions, their doctors and who to call, and a QR code. Your patient fills it in, prints it or carries it, and it stays free after the trial. Nothing on it is verified by SeniorThrive.

Before you share SeniorThrive
The four things you would want to know first
- What it costs your patient
- A family member starts free, with no credit card. The 14-day free trial starts only when the older adult joins, and nothing is charged before it ends. After that, the Emergency Card and past Room Safety Check reports stay free. New Room Safety Checks, Planning Ahead, the daily living checks and family sharing are part of Essential at $29 a month for the older adult and one family member.
- Nothing is sent to your practice. Your patient prints or shares what they choose.
- What happens to their information
- We do not sell their health information, and it is never used to advertise to them. Sharing with family is the older adult's choice, set in Privacy & Sharing. Emergency details are always visible to their Circle so someone can act quickly.
- What we leave to you
- SeniorThrive organizes the home and the household. Diagnosis and clinical evaluation stay with professionals like you.
- Who sees inside
- Your patient decides. You see inside only if they invite you into their Circle, and they can remove anyone at any time.
