The living plan that develops with you over time

A living roadmap that connects your home safety scan, immediate fixes, daily rhythms, and core legal and medical safeguards into three clear horizons: what to stabilize this month, what to systematize this season, and what to put in place when you are ready. Emergency Wallet Card

    The ambulance ride nobody planned

Blood type, allergies, current medications, and the one phone number that matters, on a card that fits behind a driver license. Print it, fold it, and it answers the first four questions a paramedic asks. On a real card the QR opens his record for whoever is standing over him, and the footer dates the last save so nobody has to guess how old it is. Scan the one on this sample and you land back on this page: Ray is invented and has no record to open.

A page of the Care Plan sample

    For everyone helping

    Care Plan

The first day somebody other than her has to run the day

The one document that is not automatic. A family sits down and answers questions about her, and this comes back: code status and who holds power of attorney, what to do when something changes, what a good day looks like, and the hour-by-hour of her actual routine. Most families never make one. The ones who do stop having the same argument every month.

    Care Plan

    Every day

Tools for every corner of daily life: health, home checks, appointments, and routines

As you use SeniorThrive, each area of your routine generates clear summaries you can hand to a physician, share with a contractor, or review together at home.

    Home Safety Report

The visit where everyone finally agrees on what to fix first

Room by room, what was found, why it matters, and what has already been done about it. Built from photos taken on a phone in about ten minutes, with the photos printed alongside the findings so nobody has to take our word for what the bathroom looks like.

    Thrive Summary

Ten minutes with a physician who has not seen her since last year

Ninety days on one page: mobility timings with the prior reading beside them, which daily tasks got harder and when, adherence as logged, vitals as ranges. Facts and dates only. The interpreting is the physician's job and the page leaves room to write.

    Balance & Fall Notes

When the doctor asks whether she has fallen this year

Every fall, slip and dizzy moment she recorded this summer, oldest first: where it happened, what she was doing, how bright it was, and how she felt afterward, with her own note beside each one. One entry was added by her daughter after a phone call, and the page says so. It records what happened and stops there. What it means is for the doctor to decide.

    Medication Discussion Notes

The appointment where he asks whether he still needs it

What the published labeling says a medicine is for, what it says to watch for, and three questions worth asking. It takes no position on whether the medicine is right for anyone. That gets decided in the room, which is the point of bringing it.

A page of the Driving Check Report sample

    For the conversation nobody starts

    Driving Check Report

The year you notice he stopped driving to the coast

Every answer as it was given, added up into a questionnaire total, with the road awareness exercises counted separately and never folded in. It says in red what it is not: a record of what was reported, not a rating of how he drives, and not a fitness-to-drive determination. Only a physician or the state can make one. What a family gets is something specific to talk about instead of a standoff.

    Pet Care Card

The hospital stay that starts at 2am

Feeding, medication, the vet's number, which shots are current and when the next one is due, and the things only she knows: he is deaf in one ear, and he will tell you he has not been fed. Independence includes the part of her life nobody else thinks to write down.

    Planning ahead

Planning ahead with love, clarity, and peace of mind

A page of the Care Preferences sample

    For the hospital hallway

    Care Preferences

The decision somebody has to make while she cannot

What she wants, in her words, written down while she is well. The cover says NOT AN ADVANCE DIRECTIVE in large letters because it is not one: it is the conversation you bring to the doctor who fills out the real form. Most families discover that difference at the worst possible moment.

    Funeral and Memorial Wishes

The week nobody can think straight and decisions cannot wait

Burial or cremation, where, who speaks, and whether a plan is already paid for. That last one matters more than it sounds: families buy a second plot every day because the paperwork for the first one was in a drawer nobody opened.

    My Final Wishes

    How these were made

Each file below came out of the same place yours would, run against a real-life household scenario. Nobody designed these pages as marketing mockups and nobody touches them up. When the real thing changes, these get rebuilt and change with it.

My Home, Step by Step is the living roadmap that organizes room check findings into three clear action horizons: what to stabilize right now, what to systematize this season, and what core safeguards to keep ready in your vault. As you complete steps or scan new rooms, it updates with you.

The rooms in the Home Safety Report are generated images of a house that does not exist. A real report is assembled in the recipient’s own browser, and photographs of the inside of someone’s home never travel through our email or leave with a marketing team.

The care plan is the one that is not automatic. Nothing produces it in the background: a family answers a thoughtful set of questions about the person, and SeniorThrive organizes the plan back from those answers. The sample is one family’s plan, not a generic template, and yours will read specifically to your family because the answers are yours.

Where a document states something about a medicine, it quotes published labeling and says so. It does not tell you whether a medicine is right for anyone. That conversation belongs to the person and their prescriber, and these pages exist so you can walk into appointments prepared and confident.

We are not a PDF creator. We are your living partner at home.

None of these documents are just static paperwork. First and foremost, they give you quiet confidence and total clarity over your own home and health. From there, they become tangible proof you can hand to family, doctors, or anyone in your circle of care.

When you work your plan, you get useful tools to leverage in your life and take with you to share with the people who matter. They are the tangible proof of your progress—clear, organized, and always yours.

You are building the living support system that helps you thrive at home. If it is your life, it stays yours and you decide who sees it. If you are helping someone you love, it stays theirs.

Ready to see what progress looks like for your home?

Start with one simple step, like a quick room safety check or your living home plan. Steady peace of mind, built step by step.

No credit card required. Look around at your own pace, and invite family whenever you are ready.
