    In Texas, independence is a core value. That is exactly why the Consumer Directed Services (CDS) option is so popular across the Lone Star State.

    Whether you are on STAR+PLUS, CLASS, or the HCS waiver, Texas Medicaid allows you to choose the CDS Option. This doesn't just give you a say in your care; it makes you the Employer.

    You hire, you train, and you manage your own attendants. Often, those attendants are your own family members or friends.


    How CDS Works in Texas (2026 Edition)

    In Texas, the "CDS Employer" (the senior or their representative) is responsible for the business of caregiving.

    The Essentials:

    • Pick Your Team: You hire who you want. In Texas, you can hire family members (except for a legal spouse or a parent of a minor).

    • Select an FMSA: You choose a Financial Management Services Agency. They aren't your boss; they are your "back office." They handle the payroll, tax filings, and background checks.

    • Master the EVV: In 2026, Texas strictly enforces Electronic Visit Verification (EVV). Your caregivers must clock in and out digitally to ensure Medicaid compliance.

    The "Hidden Work" of a Texas CDS Employer

    Being the boss comes with a catch. In Texas, a CDS Employer is solely responsible for:

    1. Backup Planning: If your caregiver doesn't show up, there is no agency to call. You must have a pre-arranged backup plan.

    2. Training: You are responsible for ensuring your caregiver knows how to handle transfers, medication, and hygiene safely.

    3. Compliance: If the EVV isn't managed correctly, payments can be delayed.

    Texas funds the employee. It does not provide the safety net.
