    Vital Signs — Your Complete Guide

    Vital Signs is where you record your key health measurements — like blood pressure, heart rate, and weight — so you and your care team can monitor your health and spot any changes over time.

    What to Expect

    • Select which vitals to monitor
    • Log readings like blood pressure or weight
    • View trends and stability indicators
    • Share your history with a provider

    Finding Vital Signs

    1. Sign in to your SeniorThrive account.
    2. Look at the menu on the left side of the screen.
    3. Find the My Health section and click on it.
    4. Click Vital Signs. The Vital Signs page will open.

    What Can You Track?

    Vital Signs has seven tracking cards, organized into two groups.

    Core Vitals

    • Blood Pressure — the force of blood against your artery walls (recorded as two numbers: Systolic and Diastolic)
    • Heart Rate — how many times your heart beats per minute
    • Temperature — your body temperature in degrees Fahrenheit
    • Respiratory Rate — how many breaths you take per minute
    • Oxygen Level — the percentage of oxygen in your blood

    Metabolic & Body Measures

    • Weight — your body weight in pounds
    • Blood Glucose — your blood sugar level in mg/dL

    Each card shows your number of past readings and has two buttons: History (to see past entries) and Add (to record a new entry).

    How to Log a Reading

    Every vital sign follows the same basic steps:

    1. Find the card you want to update and click the blue Add button.
    2. Under "When did you take this reading?", click Just now or Pick a date.
    3. Enter your measurement in the number box.
    4. Optionally click Add context or notes to type a short comment.
    5. Click Add Reading to save.

    Blood Pressure

    Blood pressure is recorded as two numbers: the top number (Systolic) and the bottom number (Diastolic).

    1. Click Add on the Blood Pressure card.
    2. Choose your date.
    3. In the Systolic box, type your top number (e.g., 120).
    4. In the Diastolic box, type your bottom number (e.g., 80).
    5. Optionally add a note.
    6. Click Add Reading.

    Tip: Blood pressure is written as two numbers separated by a slash, like 120/80.

    Heart Rate

    1. Click Add on the Heart Rate card.
    2. Choose your date.
    3. Type your heart rate in bpm (beats per minute). A typical resting rate is 60–100 bpm.
    4. Click Add Reading.

    Temperature

    1. Click Add on the Temperature card.
    2. Choose your date.
    3. Type your temperature in °F. Normal is around 98.6°F.
    4. Click Add Reading.

    Respiratory Rate

    1. Click Add on the Respiratory Rate card.
    2. Choose your date.
    3. Type your breathing rate in breaths/min. Normal at rest is 12–20.
    4. Click Add Reading.

    Oxygen Level

    1. Click Add on the Oxygen Level card.
    2. Choose your date.
    3. Type your oxygen reading as a % (e.g., 98). Healthy is typically 95% or above.
    4. Click Add Reading.

    Weight

    1. Click Add on the Weight card.
    2. Choose your date.
    3. Type your weight in lbs.
    4. Click Add Reading.

    Blood Glucose

    1. Click Add on the Blood Glucose card.
    2. Choose your date.
    3. Type your blood sugar in mg/dL.
    4. Optionally add a note (e.g., "Fasting" or "2 hours after lunch").
    5. Click Add Reading.

    Viewing Past Readings (History)

    1. Find the card you want to review.
    2. Click the History button.
    3. Your past readings will appear with dates and times.

    Changing the Time Range

    1. Look for the Time Range bar near the top of the page.
    2. Click Last 7 Days, Last 30 Days (default), or Custom.

    For custom: click Custom, set your start and end dates, then click Apply. Click Clear Filter to return to the default.

    Sorting Your Cards

    Click the up-and-down arrows icon at the right end of the Time Range bar to reorder your cards.

    Choosing Which Vitals to Display

    1. Click Choose What to Track near the top of the page.
    2. A panel will show two groups: Core Vitals and Metabolic & Body Measures.
    3. Click any card to hide it. Use Select all or Clear all as needed.
    4. Click Done.

    Note: Hiding a card only removes it from view — it does not delete any saved readings.

    Sharing Your Vital Signs

    1. Click the Share button near the top of the page.
    2. Choose who to share with: Family Members, Caregivers, or All Circle Members.
    3. Click Share to confirm.

    Downloading or Printing Your Vital Signs

    1. Click the Download/Print button.
    2. Review the PDF preview.
    3. Click Download PDF or use the print icon.
    4. Click Close when done.

    Frequently Asked Questions

    What does "0 readings" mean?
    You have not logged any entries for that vital sign within the selected time range. Click Add to record your first reading.

    Can I log a reading from an earlier date?
    Yes. Choose Pick a date instead of "Just now."

    Where do I find my blood pressure numbers?
    They appear on your blood pressure monitor. The top number is Systolic; the bottom is Diastolic.

    Do I need a special device for oxygen level?
    Yes — a pulse oximeter, a small clip-on device for your fingertip. Available at most pharmacies.

    What if I entered the wrong number?
    Click History on that card to find and edit or remove the entry.

    How do I get help?
    Click Get Help in the top right corner, or click the ? circle next to the "Vital Signs" heading.
