When paramedics, emergency room staff, or a family member needs to know your medical history, allergies, or who to call — this page makes sure that information is ready and waiting. Taking a few minutes to fill this out completely could make a real difference when it matters most.
Your information is saved automatically as you type, so you never have to worry about losing your work.
How Your Information Is Kept Safe
Your emergency information is private. It is only visible to you and the people you personally choose to share it with through your ThriveCircle. SeniorThrive uses bank-level, HIPAA-compliant encryption to protect your data, and your personal health information is never sold to advertisers or anyone else.
Filling Out Your Emergency Profile
The page is organized into five sections. Each section can be expanded or collapsed using the up and down arrows. You can also use the Expand and Collapse buttons near the top of the page to open or close all sections at once.
Personal Information
Your name, date of birth, phone number, address, and email are pulled in automatically from your main profile to save you time. Because this information will be used in an emergency, please take a moment to review it and make sure everything is accurate and current.
Medical Information
This is one of the most important sections to complete. It gives emergency responders and healthcare providers the critical details they need to treat you safely.
- Blood Type — Select your blood type from the dropdown if you know it.
- Allergies — Three separate fields: Medication Allergies, Food Allergies, and Environmental/Other Allergies. If you are unsure of an allergy, leave it blank.
- Current Medications — List all medications you take. You can also import this list directly from your Medicine Tracker using the Import button.
- Known Medical Conditions — Enter any diagnosed conditions such as diabetes, high blood pressure, heart disease, or asthma.
- Past Surgeries & Implants — List any surgeries or implanted devices. Include the year if you remember it.
- Mobility & Assistance Devices — Check any devices you use regularly: Walker, Cane, Wheelchair, Hearing Aid, Oxygen, or None.
Emergency Contacts
Emergency Contact 1 is your primary contact — first responders will call this person first. Fill in their first name, last name, and phone number. This field is required.
Emergency Contact 2 is optional but strongly recommended. Use the Copy to Contact 2 button to quickly copy details from Contact 1 as a starting point.
Take care to make sure the phone numbers you enter are current.
Care Team & Preferences
- Primary Care Doctor — Enter your doctor's name and phone number.
- Add Additional Care Team Member — Use this button to add other providers.
- Preferred Hospital — Enter the hospital where you would prefer to receive care.
- Preferred Pharmacy — Enter your pharmacy's name so your medication history can be accessed quickly.
Insurance Information (Optional)
Enter your Insurance Provider, Member ID, and Policy Group # so this information is available without you having to search for your card in a stressful moment.
Sharing and Printing Your Emergency Information
Once your profile is complete, the Share & Access Options section gives you three ways to make sure this information is accessible:
- Share with your ThriveCircle — Sends your emergency information to family members or caregivers in your ThriveCircle.
- Download Full PDF — Creates a complete printable document. Consider keeping a copy on your refrigerator or in a kitchen drawer.
- Wallet Card (3.5x2") — Downloads a compact card sized to fit in a standard wallet with the most critical details for first responders.
Tips for Keeping Your Information Current
- Review your Emergency Info at least once a year, or any time your medications, conditions, or contacts change.
- After any new diagnosis or surgery, update your records right away.
- If a loved one helps manage your health, share your emergency profile through your ThriveCircle.
- Use the Import button in Medications to keep your list in sync with your Medicine Tracker.
